But now, it is annoying. Also, how did he get to the point, before luffy and moocy?

 I remember him somewhere stuck with some people, to fight and now he is leading.

When he lost his consciousness, his alter ego surfaces. The same isn't applicable for usopp and bortolomeo, madara.
Cavendish didn't know the existence of his alter ego.
Where as, usopp and bortolomeo are "acting" like some other person trying to impress others.

You've got Multiple Personality/Split Personality disorder mixed up with "alter-ego"...........Alter-ego just means that a person has another personality, irrespective of whether he knows it exists or not..........For example, Spider-Man is the alter-ego of Peter Parker, Superman is the alter-ego of Clarke Kent, Bruce Wayne is the alter-ego of Batman, etc..........
